Hart Family Medicine of Spokane provides primary care services as here defined: “Primary
care means routine health care services including screening, assessment, diagnosis, and
treatment for the purpose of promotion of health, and detection and management of
disease or injury.”

*Fracture Care and casting is subject to the comfort level of the physician. Depending on the complexity of each specific case, the physician may, at his own discretion, elect to refer the patient to a specialist for management. Management by specialists is not a covered expense.
· Diagnosis and initial care will be included in the member plan.
** Minor skin surgeries are defined as epidermal and dermal and include such lesions as small nonmelanoma skin cancers, small epidermal cysts, and small sebaceous cysts. Some skin lesions however, may be referred to specialty care depending on the size, location, and complexity of the required removal. Whether the physician elects to include the services as part of the member plan or refer them to a specialist will be at the sole discretion of the physician. Initial diagnosis which may also include a sample biopsy would be included in the plan. Removed or biopsied specimens may, at the discretion of the physical be sent to the pathology lab for identification. Because the lab company is separate from Hart Family Medicine, their charges are the patient’s responsibility.
