Q: Is On Stage Health an insurance
plan?
A:
No, On Stage Health is a membership plan offered by primary care
physician groups that provide primary care medical services.
Q: What is the Deductible amount?
A:
There is no Deductible amount.
Q: What is the visit Co-Pay amount?
A:
There is no Co-Pay amount.
Q: What is the percentage of Co-Insurance that I
have to pay for my On Stage Health services?
A:
There is no Co-Insurance amount due from the member. On Stage Health is
not an insurance plan.
Q: When will my checking account or credit card be debited for my
membership fee?
A:
You will be debited for the $15 per person enrollment
fee and the monthly membership fee when you first sign up for membership. After that first payment, your account will be
debited for the membership fee on the 25th of the month for checking accounts and the 27th of the month for credit/debit cards.
Q: Can I enroll in On Stage Health if I have a
pre-existing condition?
A:
Yes. Please note that On Stage Health provides only primary care services. Please call the Physician Group you have selected directly
for additional information or if you have specific questions.
Q: When can I make an appointment with my Physician
Group?
A: After you receive confirmation of your membership, we recommend that you call for an appointment to establish care with your physician. Your membership is effective the first of the month after you enroll unless special arrangements are made to begin immediately.
Q: How long is the contract period for me if I
enroll in On Stage Health?
A:
After enrollment, you remain a member of your Physician's Group's On Stage Health plan by monthly payment until you cancel your membership.
Q: How do I stop being a member in On Stage Health?
A: You may contact our "cancelling" email address: cancel@OnStageHealth.com or write us at On Stage Health, 2910 E. 57th Street, Suite 5 #105, Spokane, WA 99223. If you would like your membership canceled before the end of the month, let us know the date and your unused pro-rated membership fee will be returned to you. Otherwise your membership will cease at the end of the month. We will cancel your membership with your physician group and refund any pro-rated amount to you by check within 10 days.
If you choose to change your doctor group, this action is not considered a cancellation.
Q: Can I re-enroll in On Stage Health?
A: Yes! You are welcome to re-enroll in On Stage Health. Your re-enrollment
registration fee is 2 months of your new monthly membership fee plus the
regular monthly fee for the following month. You then pay the regular
monthly dues for the physician group from whom you would like services. Occasionally a medical group will have a different re-enrollment guideline. The group guides this process.
Q: Can I choose my physician?
A:
Each physician group has their own guidelines for this process.
Generally you are able to do so as long as the physician is available for new patients.
Q: What medical services are included in primary
care medical services?
A:
Primary care means routine health care services including screening,
assessment, diagnosis, and treatment for the purpose of promotion of
health, and detection and management of disease or injury.

Q: Are lab and radiology tests / interpretations
included in my monthly membership fee?
A: Some medical groups include laboratory tests performed in their offices; some have menu based fees for lab and imaging services. Test interpretations are not
included and are either payable by you or by your insurance plan. See the medical group's Member Services Guide.
Q: Am I required to have an insurance plan if I am
an On Stage Health member?
A:
No. Note: If you do have an insurance plan, it may
cover services that are not included in your Direct Care program.
(On
occasion a physicians group may require insurance - check member
services guide for details.)
Q: What happens if I receive care from or need a
specialist physician?
A:
Services received from a specialist physician are not included in the On Stage Health program. If you
receive care from a specialist physician their services are not
included in your membership plan. You may pay for their
services or if you have insurance,
the specialist may bill your
insurance plan for their services.
Q: What happens if I receive hospital care or need
care in a hospital?
A:
Services received from a hospital are not included in the On Stage Health program. If you receive
care from a hospital, their services are not included in your membership plan. You may pay for their services or if you have
insurance, the hospital may bill your insurance plan for their services.
Q: What other services are not included in my On Stage Health membership?
A:
Acupuncture, chiropractic care, cosmetic surgery, dental care, drug and
alcohol dependency treatment. emergency room care, hearing care, home
health care, hospice care, infertility services, marital and family
counseling, massage therapy, maternity/prenatal/labor/delivery services,
mental health care, physical therapy, rehabilitation services, sexual
dysfunction, sterilization services, vision care, and weight loss
treatment.
Q: What does On Stage Health do?
A:
On Stage Health performs administrative services for physician groups
offering On Stage Health membership plans. On Stage Health also provides a web site
for members, physician groups, insurance, benefits brokers and the general public and other
related services.
