Updating Your Information
If you would like to change or update your information, please log into your account or contact us via phone or email.

Canceling Your Membership
To cancel your membership, simply do the following:

• Send an email to Cancel@OnStageHealth.com and let us know when you would like your membership cancelled.
• We will cancel your membership with your physician group and if cancellation is prior to the end of the month we will refund any pro-rated amount to you by check within ten days.
